Test case:
* https://github.com/rolfbjarne/TestApp (branch: debugger-crash / hash: 249b57f24e0bc68c0ea2765c7255880c3f017555).

To repro:
* run in simulator (haven't tried on device).

Result:

Notification: NSConcreteNotification 0x7ad39a20 {name = _NSThreadDidStartNotification; object = <NSThread: 0x7a829e70>{number = 10, name = (null)}}
error: * Assertion at ../../../../../mono/mono/mini/debugger-agent.c:2475, condition `tls' not met

Notes: 
* It only happens with SGen.

Fixed in mono master 53406a1e0c22815315939d1f2b3a38efc9f75ce4/mono-4.0.0-branch ea43208df619e352ec46f3e35d1a8271db2b1b2a.
