In the sample app provided, the top two buttons perform synchronous web service calls which work fine.
The third button initiates an asynchronous web service call to the same service, but the completion method never gets called.
After investigation, I realized that a call to a method using SoapHttpClientProtocol.InvokeAsync() is not working, the method is never called. I checked with SoapHttpClientProtocol.Invoke as well and also the method was never called. The method does exist and calling it outright works. 

The relevant call is made on line 178 of the Reference.cs file:
this.InvokeAsync("GetTienduizendRelaties", new object[0], this.GetTienduizendRelatiesOperationCompleted, userState);
