If I install a old version of an apk on to a device or simulator, visual studio on the MAC will not override the existing apk package with a newer development version.
Xamarin Studio will override it.
Build an apk in Visual Studio for MAC
drag and drop the apk on the device or simulator
make changes to the existing code. Increment the version numbers for Android
switch to debug mode.
The versions do not get updated. The code runs from Visual Studio MAC, but the "code" is never executed until I remove the old apk from the device.

Marking this bug as CONFIRMED as I can replicate this behavior. This is a historic issue that goes back a few years:
It seems that there is an issue with either files being copied over, or adb not seeing certain files being changed
