I'm unable to set any breakpoints most of the time - occasionally, I am able to set them, but most of the time not. (Version 3.1 - not in dropdown above)
What about version 220.127.116.11?

No need to wait a few days - back to its old behaviour of not allowing me to set breakpoints - it also won't allow me to remove one which was set when it was working properly.

For what it is worth I see this often. The All the breakpoint items in the 'Run' drop down menu become disabled. I will try to contribute to hunting this down.
I'm facing this issue almost constantly.
XS 4.0.8 (build 2)
Same here. All the menu items wrt breakpoints are disabled. This occurs regardless of the type of project (exe/lib) or the selected build configuration and platform (iphone/iphonesimulator), or whether the app is currently running or stopped.
There is no need to create a sample project. I just created a new solution of type iPad -> Single view application. Could not set any breakpoints. Built it - no difference. Ran it on the simulator - still no breakpoints. Since I normally use Visual Studio, I don't know when this stopped working on Xamarin Studio.
I'm on XS 4.0.8 (build 2) running on OSX 10.8.4.
This has been fixed in 4.0.9 in the Beta channel
