open a file manually in monodevelop (not part of the current project).
edit file with another editor.
go back to monodevelop: the file is now out of sync instead of prompting for "reload from disk or keep changes".

This doesn't happen if the file is part of the project. 

Feature request:
A related feature request is to have an option for setting a default for "reload from disk" or "keep changes", instead of prompting the user each time.
