This might be related to bug 2819, but I get errors that seems new in 188.8.131.52 so I'm filing as new. If you investigate and see that it's duplicate, feel free to move.
Steps to reproduce:
Build an MFA app
Hit run, then start an emulator
Once emulator is fully loaded this info doesnt refresh (bug 2819)
1) Expected result
Every time, it will kill the adb server and restart it, populating the select device window with available devices
2) Actual result
Sometimes clicking refresh will error out with one of these errors
If you try to reproduce this by clicking refresh more than a couple of times, you will probably run into bug 1734.

The third (first, time wise) error in the gist above is actually the result of bug 3496.
I can repro the running emulator not showing up after it has been started. I think it's caused by us querying the emulator before it has initialized which results in an exception being thrown and we fail to populate the list. I'll need to investigate further.
I committed a fix in master and cherry-picked to 2.8.8-series.
3.0 monodevelop plugin for VS has this behaviour now. Always blank Select Device Dialog.
