I've been redirected twice when filing this bug and I hope I'm finally in the right place (https://github.com/i3/i3/issues/1762, https://sourceforge.net/p/keepass/bugs/1396/)

Using Debian Stretch (i3 v4.10.2), mono v3.2.8 and keepass 2.29:

It seems that mono doesn't handle tray icons properly. From other bug reports, the person helping me there suggested that mono doesn't properly handle the XEMBED info, which apparently is needed for the tray icon and the keepass itself can't do that.

The result is either no tray icon at all, or this odd scenario (http://i.imgur.com/DjmHiYZ.png), which could not be reproduced by me.
