It is hard to reproduce, but i faced it by these steps, not all of which maybe relevant

1) Make changes to a file
2) Right click on the file and click Revert
3) When the confirmation window comes up, press spacebar (because I thought maybe if it works on Windows, it should also on Mac)
4) Nothing happens
5) Press Enter

and then the revert failed. The changes were still in the file

MonoDevelop.Ide.Gui.Document.Reload () is not thread safe. It needs to be called on the UI thread.
