The Monodevelop GTK theme under MacOS X is ugly, (not only but mostly) because it doesn't match the rest of the system's UIs - not in the slightest.
The guys from the Banshee team made some improvements to the default mono GTK theme: http://abock.org/2010/01/29/banshee-1-5-3-and-the-return-of-os-x-support
Banshee doesn't look native either, but it looks better than the default theme.
It might be a good idea to get together with the banshee team and port this over (and maybe improve) to monodevelop or maybe even mono itself?.
We'll be focusing more on this type of thing soon, probably right after the 3.0 release.
This was fixed in MD/XS 4.0.
