When converting an app to 1.3.1 iOS unified API if you have microsoft BCL in your references the iOS resources do not get copied into your app bundle
I was having the same issue (converting to Unified API + Xamarin.Forms 1.3.2 in my case). Also, the icons are no longer associated with the app so it shows the default "empty" icon on the homescreen.
My icons were in an icon asset catalog. After removing the BCL stuff and doing a clean build, the images were back, but I had to go back to "Don't use asset catalog" and add the icon files again by hand. After that, I could return to the asset catalog again.
This issue wasn't related to forms but was already fixed, so im closing the issue for now, please reopen if you still have this problem.
