I'm using the latest stable as of today, 6.4.1 and Mono 3.2.0.

I do a Build/Clean All then Run/Upload to my device, all works fine.

I make a few changes in a class, delete the app from my device, do a Run / Upload to device

app crashes on startup

If I do a Build/Clean All then run/upload to device, it will work again.

It seems there is a problem with your build system where it's not doing a full clean/rebuild and something is getting stale and causing an issue.

another issue... just arose.

I did the build/clean all, uploaded to iPhone 5 and app ran.

I deleted the app

again click Run/Upload to device after NO changes in code

app crashes on launch

Something is stale somewhere after publishing to device or causing corruption.

Can you attach the crash reports for the crashes you get? However I'll likely also need to get access to your app in order to reproduce the crashes myself.

I'm not sure there any reports.  I upload to device and it crashes on launch.  I'm going to be working on another app today, I'll see if it happens with that app as well to see if it's an app specific issue or not.

Every crash should generate a crash report - but if you can't find one there should also be more information in the device log.

It just happened again, it's fairly reproducable it seems.  I don't know where to get any information to help you.  I looked a Device Logs in Xcode for this iPhone and nothing is listed.  If there is something I need to enable or do to collect information for you please dumb it down to my level and tell me what I can do to help you resolve this.

I'm using RELEASE config on my solution which is Link SDK only, LLVM OFF, Optimize PNG, ARMV7, Enable Generic Value type sharing.  i don't have anything beta installed regarding Xcode.
