Once clicking anywhere within the MD application, mouse clicks have no effect after the initial click. This is true within the navigation tree or the code editor.
Moving focus to another application and back, or selecting an option from the MD System Menu allows for ONE click within MD.
I finally realized that with MD in "full screen" mode (ie. the menu not visible) the mouse always seems to respond as expected.
Seems to be similar to "Mouse clicks on dialogs annd menus sometimes have no effect unless MD is defocused and refocused." as described @ http://monodevelop.com/Developers/Mac_Support.

This part sounds really familiar to what is fixed by the patch in bug 546 comment 19.
It would be great if this could be re-tested once a release is out with a newer GTK+ which contains the patch form bug 546 comment 19. I do not seem to have permission to set a dependency on bug 546.
This should be fixed by the GTK+ version that's currently in Mono beta, so I'm marking it fixed. Please re-open if it happens with GTK+ version is 2.24.8 or later. GTK+ version can be found in the MD About dialog.
