I'm running Ubuntu 12.04, with mono from badgerports repo and pmono. I downloaded MD 4.0 from official download page.
When opening a project or file, editor does not display any thing, it is just grey page. Refer to screenshot.
Works for me on my ubuntu 12.10 - but I suppose you'll need a custom mono since the badgerports mono is a bit outdated (I recommend at 2.10.11 - but a 3.x beta runs as well)
Y I know it's hard to get an up2date mono on linux :/
I tested with pmono 3.0.1 and issue is same. :(
Might be the QT GTK theme engine (it's notoriously buggy). Could you try changing the GTK theme?
I can confirm that we've a problem with the oxygen-gtk engine.
I've tried the version from the repositories as well as the latest oxygen theme engine - both show the bug. It's a very weird behavior :/
The bug should be fixed (would be nice if you could get a newer MD4.0 - from git master)

hm, you need to use git:
git clone git://github.com/mono/monodevelop.git
and in the monodevelop directory:
git submodule init
git submodule update --recursive
(the submodule stuff should not be needed - configure/make should do it, but if compilation fails with the git version try it out)
